Patients, legal representatives, and properly authorized individuals may request PHI or medical records in accordance with law and agency policy.

Complaints and Incidents
Patients, families, caregivers, and representatives are encouraged to report concerns, complaints, privacy issues, or incidents so they can be reviewed and addressed.
